PHYSICAL HANDLING AS AN ETHICAL PROBLEM FROM NURSES PERSPECTIVE

Background: Physical manipulation provided by general nurses no matter how it may concern the bodily realm, may interfere with the ethical realm.
Aim: Find out how the nursing staff relates to the ethical approach and its implementation in practice in the physical manipulation of the patient that is the principle non- maleficence, beneficence and autonomy respect. By breaking these principles, while physical manipulation
provision by nurses, it can easily became to physical injury but also to psychical humiliation, which we can meet in praxis and we consider it as an ethical problem.
Methods: A questionnaire of own design based on the study of literary sources was used for quantitative research. One of the five areas of the questionnaire addressed the ethical approach to providing physical manipulation to the patient. The group consisted of 96 general nurses from 5
South Bohemian hospitals who received a qualification education general nurse on universities, high nursing schools and secondary nursing school up to 2007. Based on the first and second stages of classification through the SASD (Statistical Data Analysis) and SPSS programs the data analysis was performed.
Results: The results of the relationship of ethical approach of nurses in connection with the physical manipulation of patients shown in contingency and frequency tables indicate a rather positive relationship and approach toward to ethical principles. However, the respondents´
evidence from the real practice shows the observation of the principles to be impracticable. The education level plays some role in the observation of the principles. Respondents with a university degree are not significantly more likely to use a manipulation aid if a co-worker refuses to use it than respondents with a lower education (SZŠ, VOŠ) (p < 0.01). Respondents see the reasons of this contradiction in the number of personnel, lack of manipulation aids, insufficient patient´s cooperation due to the press of time and insufficient communication. Discussion: The ethical approach respondents perceive the ethical approach as important and substantial. In spite of positive relation to
ethical principles´, respondents meet some obstacles, which obstruct the provision of such approach. If they would have enough of personnel, space equipment, enough time, stronger management support, they would like to pay more attention to their patients, so that there would
be no negative patients experience the ethical principles would be observed.
Conclusion: The research survey pointed to factors that influence the predominantly positive attitude of nurses towards an ethical approach to patients in the implementation of physical manipulation. Nurses are aware of non-maleficence importance while provision of care, nevertheless
pointing on the improper working conditions, which not enable them to observe these principles ethical approach. They would need more personnel, manipulation aids, and management support as well. Further results uncover possibilities for improvement in nurses' communication,
which due to its inadequacy may affect the patient's autonomy and respect experience. We see the possible change in the improvement of qualification education and lifelong education, what means the management must be involved in to this process.
